slope intercept form = y=mx+c where m is slope and c is constant

Given that:- slope of line = 1/5 and y- intercept is -8 and we can say that y- intercept is (0,-8)

put the values in the equation of line=> 

y-(-8)=1/5(x-0)

y+8=1/5x

y=1/5x-8

hence, this is slope intercept form of the line where 1/5 is slope of line that is m and -8 is constant that is c.
